This disturbing moment implies that this is when the monster traps Skye in a prolonged illusion until Smile 2's twist ending.

Smile 2 Set Up Two Characters Most Likely To Be Smile 3's First Victim
Following the shocking conclusion of Smile 2, the filmmakers already have two characters who could likely be the first victims in Smile 3.
At this point, Skye is practically under the Smile Entity's spell. It tricks her into thinking that she killed her mother, Elizabeth (Rosemarie DeWitt), and that she was able to complete Morris's (Peter Jacobson) plan to kill the Entity once and for all. The fact that everything between the ambush and the concert is an illusion shows that the creature has had all but complete control over what Skye sees and does.

It's unlikely anything after Skye's ambush is real. Her escaping the hospital and meeting up with Morris at the end of Smile 2 may seem real. However, it isn't plausible that anyone would let Skye appear at the concert if these events did occur. Skye would have technically committed a crime by stealing and using a security guard's gun, and no amount of star power could've let this slide. However, even if Skye didn't meet up with Morris in the film's final act, that doesn't mean he wasn't waiting for her.

Considering the depth of the monster's illusion, it is also possible that Skye actually did reunite with Gemma (Dylan Gelula) earlier in the film. The Entity leads Skye to believe it was masquerading as Gemma in Smile 2 the whole time when they flee the hospital together. However, this could be another trick designed by the Entity to mess with Skye's head to make her think she's alone.
Ultimately, Smile 2 proved itself just as shocking and deceptive as the franchise's first film. Director Parker Finn defied expectations with this sequel, secretly showing when Skye lost control of her mind while still unleashing a horrifying twist ending. Though not everything that happened to Skye in the final act may not have been a lie, it doesn't make the conclusion to Smile 2 any less tragic or terrifying.
